1. Purpose of This Document

This document records the written consent of the business owner of Ideal Care Homes Bellevue ("Ideal Care Homes Bellevue", "we", or "us") to receive SMS / text-message notifications sent through a Twilio Application-to-Person (A2P) 10DLC messaging campaign operated on behalf of the business. It exists so that mobile carriers and The Campaign Registry reviewers can verify, at a public URL, that consent has been collected for every recipient of this campaign.

2. Sole Recipient

This A2P campaign has exactly one recipient: the business owner of Ideal Care Homes Bellevue. The business owner is the same individual who registered the Twilio Brand and Campaign on behalf of the business and who personally provided her mobile phone number to the website's server-side configuration.

• No website visitor receives SMS through this campaign.
• No prospective resident, family member, or applicant receives SMS through this campaign.
• No third party, affiliate, or marketing partner receives SMS through this campaign.
• There is no public-facing SMS sign-up form, keyword opt-in, or subscription list.

3. Scope of Consent

The business owner consents to receive operational text-message notifications on her personal mobile phone whenever a website visitor submits one of the following forms on the company's official website at https://idealcarehomesbellevue.com:

• The Careers Application Form at https://idealcarehomesbellevue.com/careers

Each notification identifies the brand "Ideal Care Homes Bellevue", names the visitor who submitted the form, summarizes the inquiry or application, and prompts the business owner to check her business email at info@idealcarehomesbellevue.com for the complete submission. The text messages are operational alerts only. They contain no marketing content, no promotional offers, and no third-party advertising.

4. How Consent Was Collected

Consent was collected in writing, offline, during the configuration of the Twilio messaging service on May 22, 2026. By providing her own mobile phone number for storage in the website's server-side environment configuration and by authorizing the Twilio A2P campaign on behalf of the business, the business owner has knowingly and directly opted in to receive these messages. She is both the consenting party and the sole recipient. No agent, employee, or third party has consented on her behalf.

5. Message Frequency, Rates, and Disclosures

The following disclosures apply to all SMS sent through this campaign:

• Message frequency is approximately 1 to 10 messages per week, varying directly with the volume of form submissions received.
• Message and data rates may apply, per the recipient’s mobile carrier plan.
• Mobile carriers (including but not limited to T-Mobile, AT&T, Verizon, and others) are not liable for delayed or undelivered messages.
• No mobile information collected through this campaign or the website is shared with third parties or affiliates for marketing or promotional purposes.
• The third-party messaging provider is Twilio Inc. Twilio processes the message content solely for the purpose of delivering each notification and does not use the data for marketing.

6. Opt-Out and Help

The business owner may revoke this consent at any time, in two independent ways:

• Reply STOP to any received text message. Twilio enforces opt-out automatically and permanently for the replying phone number.
• Remove the mobile phone number from the website’s server-side environment configuration. This immediately and permanently halts all outbound SMS from this campaign.

Replying HELP to any message returns help information that includes the business name and the business email address (info@idealcarehomesbellevue.com).

8. Acknowledgment

By configuring her own phone number into the messaging system and registering this A2P 10DLC campaign on behalf of Ideal Care Homes Bellevue, the business owner has acknowledged and agreed to the terms above as of the Effective Date of this document. This consent remains in force until revoked through either mechanism described in Section 6.
